O2 Employment Services is working with a local Non-Profit who is looking for a Case Manager. This is a full-time, regular role in Redding, CA.
The Case Manager will provide individualized support to youth ages 12–18, coordinating services, fostering resilience, and promoting educational, personal, and substance-use prevention goals.
Compensation of the Case Manager:
- $22- $25 per hour, depending on experience
- Vision, dental, medical offered through O2
- Benefits after probationary period: Paid Holidays (11), Paid Vacation (80 hours), Mileage reimbursement for work
Qualifications of the Case Manager:
- Bachelor’s degree in Social Work, Psychology, Education, or related field (or equivalent experience).
- Minimum 2 years’ experience in case management, youth services, or mentoring.
- Computer savvy with strong data-entry and reporting skills- Microsoft 365 Programs (Excel, Word, Teams, PowerPoint) Apricot case-management software and Canva proficiency a plus.
- Excellent organizational, communication, and interpersonal skills.
- Knowledge of local resources and commitment to community collaboration.
- Bi-lingual is a plus
Responsibilities of the Case Manager:
- Individual Case Management: Conduct comprehensive intake assessments and develop personalized case plans. Coordinate referrals to community resources (counseling, tutoring, healthcare, substance-use treatment, etc.). Monitor progress and adjust plans to meet evolving needs.
- Substance-Use Prevention & Early Intervention: Screen for risk factors and early signs of substance use. (non-clinical). Provide evidence-based education, brief interventions, and referrals to specialized support.
- Mentoring & Relationship Building: Meet with assigned youth for goal-setting, life-skills coaching, and encouragement. Facilitate group or peer to peer workshops on leadership, communication, career exploration, and healthy choices. Serve as a positive role model, exemplifying integrity, respect, and accountability.
- Documentation & Reporting: Maintain accurate case notes, progress reports, and outcome data in our case-management system. Apricot software proficiency is a plus but not required—strong computer skills are essential. Prepare monthly reports for funders and stakeholders.
- Community Engagement: Collaborate with local schools, groups, and service agencies. Represent Youth Options Shasta at community events.
- Professional Development: Stay current on best practices in youth mentoring, case management, and substance-use prevention. Attend agency trainings and share insights with the team.
